1. Treatment outcomes are most successful when therapy is attended on a consistent basis.
2. If you need to cancel or change an appointment, please contact me via text 24 hours prior to your appointment.
3. Unless there is a medical or other life emergency for you or a family member, appointments missed without notice or rescheduling 24 hours in advance are charged in full.

Insurance Reimbursement Information: 

1. All clients pay for “Out-of-Network” treatment directly after each session via an invoice that will be sent to you via Square.
2. Out-of-Network benefits are accepted by major insurance companies so please submit your receipt to them for your direct reimbursement (*See below codes).
3. For purposes of questioning your insurance carrier on their reimbursement rates, our Provider NPI number is: 1386892750 and our Tax ID number is: 260558837.
4. Diagnostic codes are established during treatment and therefore cannot be provided herein.
