Yes. I work with all insurance companies. Services are provided in the Out-of-Network provider category and qualify for all Employee Benefit Plan Flex Spending plan. I recommend asking these questions to your insurance provider to help determine your benefits:
- Is coverage full or partial payment?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
Credit card, check or cash is accepted at time of service. Monthly statements are provided for your records and CMS forms are available to help you when filing with your insurance for reimbursement. Receipts are immediately available for credit card payments.
Treatment outcomes are most successful and overall treatment time shorter when therapy is attended on a consistent basis. Unexpected conflicts however are apart of life so effort is made to accommodate life’s unexpected schedule changes. If you need to cancel or change an appointment please contact me via text, email, or phone call. Should your scheduled therapy appointment be missed without notice or rescheduling at least 24 hours in advance, please be aware that you will be required to pay the full cost of the session.
